Once Crystal sends you your Login Name and Password for TherapyAppointment.com, you will need to fill out admission and biographical information. Use the following procedure:
- Click here to get to the login page.
- The third box down on the left side is called “My Contact or insurance information has changed”. Use the Login Name and Password supplied in the secure email from Crystal. Then click “Edit my information”
- Fill in all applicable information. Insurance information does not need to be provided for Online Therapy. This would also be a good time to change your password to something you can remember easily.
- Click on “Click here when finished” to save the information. Click on the underlined “HERE” on the next page.
- At the bottom of the next page under “I want to complete a Biographical Information Form” enter the previously mentioned Login Name and Password and click “I’m ready to fill it in”.
- Fill in all applicable information in the form and when complete, click “Send this information to Mrs. Hemesath” at the bottom of the page.
